Non-Surgical Brow Lift: Botox for Hooded Eyes

Non-Surgical Brow Lift: Botox for Hooded Eyes

Key takeaways

  • A non-surgical brow lift uses small, carefully placed Botox injections to relax the muscles that pull the brow down, so the brow rests slightly higher and the eye looks more open.
  • It suits mild to moderate brow heaviness from muscle pull, not significant excess eyelid skin, which usually needs a different approach.
  • The effect is subtle by design – usually a few millimetres of lift – and over-treatment can flatten or distort the brow.
  • Results settle in 10-14 days and typically last around 3-4 months.
  • A medical assessment first matters: the wrong injection pattern can lower the brow instead of lifting it.

A non-surgical brow lift is one of the more subtle uses of Botox, aimed at softening hooded, heavy-feeling eyes without surgery or recovery time. At our Hulhumalé clinic it is often the quiet fix patients ask about after noticing that their eyes look tired even when they feel rested.

In the strong overhead light common in the Maldives a heavy brow casts more shadow across the upper lid, and squinting against year-round glare keeps the muscles that pull the brow down working harder, which is often the moment patients first notice it.

In short: a non-surgical brow lift with Botox, performed by Dr. Alaa Zidan, dermatologist, at our Hulhumalé clinic in the Maldives, relaxes the muscles that pull the brow down so it sits a little higher. It suits mild hooding, takes minutes, needs no downtime and lasts about 3-4 months.

How a non-surgical brow lift works

The brow is held in a balance. One muscle, the frontalis across the forehead, lifts it. Several others pull it down: the corrugator and procerus between the brows, and the outer part of the orbicularis oculi, the ring of muscle around the eye. Botox temporarily weakens the downward pullers, so the frontalis meets less resistance and the brow settles a little higher, most noticeably at the outer tail.

It is a modest effect. The published techniques for botulinum toxin eyebrow shaping describe subtle changes in brow position and shape, not the dramatic lift of surgery, and our approach is built around that honest ceiling. You can read how the technique is described in the literature in PubMed Central: A Novel OnabotulinumtoxinA Treatment Technique to Obtain Predictable Outcomes in Eyebrow Position and Shape.

Why placement matters more than dose

The same product that lifts a brow can lower it if it goes in the wrong place. Treating the frontalis too heavily removes the very muscle that holds the brow up, which is why the injection points for a lift sit around the outer brow and the depressor muscles rather than across the whole forehead. This is also why a brow lift is often planned together with forehead Botox: the two must be balanced, not treated in isolation.

Who a non-surgical brow lift suits – and who it does not

It tends to suit people whose brow sits slightly low or flat, whose outer brow droops a little, or whose heavy-looking eyes come mainly from strong brow-depressor muscles. A typical candidate notices that their brow position improves when they raise it with a finger, and that the lid skin itself is not heavily folded.

It is less suitable when the main problem is excess upper-eyelid skin, deep age-related brow descent, or fat pads, because Botox cannot remove skin or restore volume. In those cases an eyelid or surgical brow procedure may genuinely serve you better, and we will say so and refer you if that is the honest answer. Botox is also not appropriate in pregnancy or breastfeeding, with certain neuromuscular conditions, or with infection at the injection site.

Questions we ask at the consultation

We assess your brow at rest and in movement, compare both sides (most faces are slightly asymmetric), check your forehead lines and ask about previous toxin treatment and any eye or eyelid history. We also look at how much of the hooding is skin and how much is muscle pull. If you have ever had eyelid surgery or a droopy lid, tell us before anything is injected.

What happens during the treatment

The session is quick. The skin is cleansed, injection points are marked while you move your brows, and tiny amounts of toxin are placed with a fine needle. Most people describe brief pinpricks and no need for numbing cream. Expect small bumps that settle within the hour and a possible pinpoint bruise.

For a step-by-step look at what happens at each visit, see our Botox guide, and for the forehead and brow area specifically, the forehead Botox guide.

Results, timeline and aftercare

Botox begins to work in about 3-5 days and reaches its full effect at 10-14 days, so judge the result then rather than on day two. The lift usually fades over roughly 3-4 months, similar to other Botox areas; how long forehead results last is covered in our article on how long forehead Botox lasts.

For the first few hours stay upright, avoid pressing or massaging the area, and skip hot yoga, saunas and strenuous exercise for the rest of the day. Avoid lying face down for the first evening. In a hot, humid climate, plan to keep out of direct midday sun and wear a hat and sunglasses for the first day or two, since heat can increase swelling and bruising.

Planning around island travel

Because full results take two weeks, patients who live on other islands or are travelling for a wedding or holiday should book at least two weeks ahead, so any small adjustment can be made at a review before you fly or before photographs.

Risks, side effects and how we reduce them

Most side effects are minor and short-lived: tenderness, small bumps, a headache or bruising. The risks specific to this area are a heavy or drooping brow, an overly arched or surprised look (often called a Spock brow), uneven brows and, rarely, a drooping upper eyelid if toxin spreads to the muscle that lifts the lid. These effects are temporary and fade as the toxin wears off, but they can last for weeks. We reduce them by using conservative doses, precise placement, keeping injections away from the lid muscle and reviewing you at two weeks, when a small touch-up can correct asymmetry. The American Academy of Dermatology: Botulinum toxin therapy: Overview explains the general safety profile and why treatment should be given by a trained doctor.

How it compares with the alternatives

A surgical brow lift or upper blepharoplasty gives a bigger and longer-lasting change, and is the better choice for significant excess skin, but it involves anaesthesia, scars and weeks of recovery. Filler can add support to a hollow or flat outer brow, but it adds volume rather than relaxing muscle and carries its own risks near the eye. Skin-tightening treatments such as fractional RF can help mild skin laxity. For an overview of the evidence across approaches, see PubMed Central: Techniques of Eyebrow Lifting: A Narrative Review.

Getting a non-surgical brow lift in Hulhumalé

Cost depends on the number of areas treated, the amount of toxin needed and whether you also treat the forehead or frown lines, so we give you a clear plan after the consultation rather than a number over the phone. If you are comparing options for Botox in the Maldives, choose a doctor who examines your anatomy first, explains the limits of the treatment and is willing to tell you when it is not the right option. If hooded eyes are bothering you, send a photo on WhatsApp and Dr. Alaa will tell you honestly whether this subtle approach would help, or you can get in touch.

No, it addresses mild to moderate brow heaviness from muscle position – significant skin excess needs a different approach.

Will my eyebrows look surprised or overly arched?

Not with conservative, well-judged technique – this is exactly why subtlety and experience matter for this specific treatment.

How long do results last?

Generally 3-4 months, similar to other Botox treatments.

Where can I get a non-surgical brow lift in the Maldives?
