Acne Treatment Guidelines Updated: What Changed for 2026

The updated acne treatment guidelines from EuroGuiDerm, published in the Journal of the European Academy of Dermatology and Venereology in 2026, are a targeted update rather than a full rewrite. They focus on three practical questions: when isotretinoin should be preferred over oral antibiotics, how long antibiotics should be used, and where hormonal treatments and newer topical options fit.

What the updated acne treatment guidelines recommend

The guideline strongly recommends systemic isotretinoin, where possible, for severe papulopustular and moderate or severe nodular and conglobate acne. Oral antibiotics should generally be limited to three months unless there is a specific reason for longer. Where an antibiotic is needed, doxycycline and lymecycline are preferred over minocycline and tetracycline, and azithromycin is not supported because its benefits do not outweigh the risks, particularly antibiotic resistance. For topical retinoids, adapalene is preferred over tretinoin and isotretinoin.

Spironolactone, used off-label, is an option for women as an add-on to standard treatment, and the group also considered trifarotene and clascoterone, two newer topical agents.

How strong is the evidence, and what it does not show

This is a consensus guideline built on the trials that exist, and the authors say plainly that head-to-head studies of clascoterone, trifarotene and hormonal options against other treatments are still needed. It does not prove that any one product is better than another for every person, and it is European guidance rather than a prescription for an individual. It also does not mean antibiotics are never appropriate, only that they should be time-limited.

What this means for patients in the Maldives

Heat, humidity and sweat can aggravate breakouts, and year-round sun can darken the marks acne leaves behind, so the choice of regimen matters here as much as the guideline itself. We do not know whether every newer product named in the update, such as trifarotene or clascoterone, is available in the Maldives, so please check before assuming a treatment can be obtained locally. Our guide to acne treatment options in the Maldives covers approaches suited to this climate, and our acne and scarring service addresses the marks acne can leave behind once breakouts are controlled.

Frequently asked questions

Does this mean antibiotics are no longer used for acne?

No. The guideline still allows oral antibiotics, but generally for no more than three months, and with doxycycline or lymecycline preferred, to reduce the risk of resistance.

Should I change my acne routine because of this update?

Only if your routine has not been reviewed in some time or relies on long-term antibiotics. A doctor can assess whether a different approach would suit your skin.
