Lip Flip vs Lip Filler: Comparing Lip Plumping Options

Fuller lips can be achieved two different ways, and they are often confused for the same treatment. A lip flip uses Botox to subtly roll the upper lip outward. Lip filler adds actual volume with hyaluronic acid. They solve different problems, and picking the wrong one is a common source of disappointing results.

Both are among the more frequently requested lip treatments at our Hulhumalé clinic, which is partly why the two get confused so often.
What a lip flip actually does
A lip flip uses a small amount of Botox placed at the corners of the upper lip border, relaxing the muscle that pulls the lip inward when you smile or speak. It does not add volume – it changes how the existing lip sits and shows, giving a subtly fuller appearance especially when smiling. Results last around six to eight weeks, shorter than Botox elsewhere on the face, because lip muscles move constantly.
What lip filler actually does
Lip filler adds hyaluronic acid directly into the lip tissue, increasing volume, shape and sometimes symmetry. It is the right choice when the goal is genuinely fuller lips rather than a subtle lift, and results typically last six to twelve months. Read more on our lip filler page.
Which one is right for you
If your lips are a reasonable size already and you want a subtle, natural lift without added volume, a lip flip is usually the better starting point. If you want visible fullness or are correcting asymmetry, filler is the more direct answer. Some patients use both together for a fuller, more defined result.
What to expect either way
Both are quick, in-clinic treatments with minimal downtime. Filler carries a higher likelihood of temporary swelling and bruising than a lip flip, simply because it involves injecting volume rather than a small dose of muscle relaxant.
Avoiding an overdone result
The most common complaint about lip treatments is not choosing the wrong option, it is overtreatment – too much filler, too often, without enough time between sessions to see how the lips settle. A conservative, staged approach almost always produces a more natural, longer-satisfying result.

Frequently asked questions
Can I combine a lip flip and lip filler?
Yes, this is a common combination for patients who want both a subtle lift and added volume, though we usually recommend trying one first to see how it looks before adding the other.
Does a lip flip hurt more than filler?
Generally less, since it is a small dose of Botox rather than an injected volume. Both use topical numbing.
Will my lips look overdone?
Not with a conservative, correctly dosed approach. An overdone look is almost always the result of too much product placed too quickly, not an inevitable outcome of treatment.
How soon can I get more filler if I want it later?
Typically we wait at least two to three weeks to assess the final result before considering any additional treatment.
